Homeowner Affidavit Forms:

An owner-occupant of a single-family residence may act as their own general contractor provided they currently reside in, or intend to reside at, the premises.

If owner-occupants are performing the mechanical, electrical or plumbing work, an owner affidavit form is required for each trade. Affidavits state the owner is knowledgeable of code requirements and will be performing the installation.  If a contractor is hired, the contractor will need to provide proof of licensure to the City and be present for inspections.  Affidavits and corresponding worksheets (i.e., mechanical, electrical and plumbing worksheets) are required prior to permit issuance.

Demolition Worksheet - Water and Sewer

Prior to the demolition of any structure in the City of Evanston, the contractor must disconnect both the water and sewer services at the respective mains. This worksheet must be filled out and submitted with the building permit application.

Electrical Worksheet

If there is electrical work at the job site, this worksheet must be submitted prior to permit issuance.
